Interscholastic Sports at RBV
Every student athlete needs to have these forms completed and turned into the Athletic Director, Mr. Marty Nellis, before the start of the new sport season. The Athletic Clearance forms consist of : Parent Signature page - Health History form - Student Athlete - Parent Agreement and Physical Examination (must be completed by a Medical Pesonnel)
