Required Document: TDAP - Proof of Immunization

You must have proof of a Tdap Vaccination on record in the RBVHS Health office before student registration! Students and parents can provide proof at any time! Read More about the new law in English or Spanish.

Where can I get an immunization?
	Please read this flyer printed in English and Spanish for directon.
How can you provide verification?
	You or your Health Care Provider can FAX Proof of Immunization to Anna Weirather at (760) 598-7062
	Send an e-mail with a scanned copy of the original documents attached to Anna Weirather.
	You can drop off a copy of the student immunization record in the Health office.
